I'll answer as many questions as I can. The ones I cannot answer I will forward to Bill.

1) So long as the QSOs on the application are in the same order as the cards, that is all that matters.
For Online applications, we do not need the cards to be in band and mode order.
They just need to match the order of the cards.

 3) As of April 2nd, we no longer follow the "1st application of year, 2nd application of year" rule. All applications cost the same. That is to say, all Online applications cost the same, all LoTW applications cost the same, and all Traditional paper applications cost the same (keep in mind that each of these specific applications have different fees:  http://www.arrl.org/files/file/DXCC/Fees/2012%20Fee%20Chart%20ver%207.pdf
